In my mythtv box I've got one from Ahanix...which is just a rebrand of a
company called silenx...  works pretty silently.  With some brushless fans
in the case all you can really hear is the Seagate hard drive (which I got
because it was supposedly the quietest - it is about 140 times quieter than
the western digitals I have in my other computer, but it is still the
loudest part of the pc.

I remember someone at Installfest mentioning the need to find themselves 
a quiter power supply (was that you Tim?)..  Anyway, Tom's Hardware just 
ran an article reviewing about 15 units that touches on the quiet 
factor.  I'd have read it by now if I could concetrate over the noise of 
all these PCs :)
